Should you have any communication or access needs such as large print documents, a British Sign Language interpreter, require the use of a wheelchair ramps etc, please do not hestitate to let one of our reception team know and we will endeavour to meet your needs as promptly as possible.
The Pulteney practice is working in conjunction with Masta to provide a comprehensive travel clinic for registered and non-registered patients. Clinics will run Tuesday evenings between 4pm and 7pm, Thursday afternoons between 1pm and 6pm and Saturdays between 9am and 12pm.
Whether you are travelling for business or pleasure, you will need to ensure you are up-to-date with your travel vaccinations. A health brief can be produced online, free of charge, by visiting www.masta-travel-health.com offering tailored travel advice. There will be a charge for advisory only appointments, please enquire on the number below.
You will be responsible for checking your own immunisation history and bringing this along to your appointment. This can be obtained by your GP or nurse.

The primary care trust now allow patients to self-refer for minor Podiatry or Physiotherapy. We have provided the links below to allow you to download the appropriate referral form. Completed forms should be sent directly to the service provider stated on the form. Forms received at the surgery will not be able to be actioned.

Once a patient is fully registered for NHS services, online services can be applied for. These services currently include:
To register for online services, you will be asked to complete an application form and provide photographic identification (e.g. Driving Licence, Passport etc.) for verification purposes. The whole process will be explained to you and you will then be given a user name and password. You will need to visit the surgery to do this but the process should only take about 10 minutes. Your details will be personal to you and cannot be used for anybody else.
