Shared Care Agreements

Due to a lack of identified funding, The Pulteney Practice is unable to accept Shared Care Agreements (SCA) to start prescribing or monitoring new medications.  This includes requests from the NHS as well as from private providers.  BSW (Bath and North East Somerset, Swindon and Wiltshire) GP Practices are not funded to prescribe or monitor medications as part of SCAs.  The Pulteney Practice are at capacity with our current caseload and are unable to enter any new SCAs which are unfunded.  This includes, but is not confined to, examples such as ADHD medications; Gender Affirming Hormones; and Testosterone for HRT.  We will need to DECLINE requests to enter new SCAs with both private providers and NHS providers.

Patients current on SCA medication:

If a SCA has already been signed by The Pulteney Practice, we will continue to honour it.  Please note, for a SCA to continue, the patient should have at least an annual review with their specialist.  Failure to complete an annual review by the responsible specialist will result in the SCA being terminated.

If a SCA was signed by a previous Practice, and we (Pulteney) have been prescribing medication, we will continue to honour it, providing there is ongoing follow up with the specialist, including at least annual reviews.

If there is no SCA on file, and we have been prescribing medication, we will continue to provide a service, providing an annual review with a specialist has taken place AND an updated SCA has been agreed within 6 months.
