IT/Electronic patient records – statement of intent for The Pulteney Practice
Contractual requirements came into force from 1st April 2015 requiring that GP practices should make available a statement of intent in relation to the following IT developments:
- Referral Management
- Electronic Appointment Booking
- On line Booking of repeat prescriptions
- Summary Care Record
- GP2GP transfers
- Patient Access to the detailed information from the medical record.
- Full Clinical Record Access
The Pulteney Practice now offers to our patients all of the requirements under the GP contract relating to IT developments and are working closely with our Clinical System Suppliers to allow patients to access the detailed information from their medical record.
Patients can now access their Detailed Coded record information and their Full Clinical record information online, to do this you will need an online account (please note Full Clinical Record access includes Detailed Coded Record access). Your request for Detailed Coded Record access or Full Clinical Record access will take up to 14 days to process.
To acquire a username and password for an online account please visit the Surgery Reception. For your own security you must bring some form of photo ID.
If you are a carer, you will need to provide written permission from the person you care for before we can give you their logon details.
Parents should note that all patients aged over 16 will need to attend in person with photo ID and will be given their logon details individually. We will not provide logon details to the parents of any children aged over 16 unless the teenager is present with their photo ID.
Once you receive your log in details, we strongly recommend that you read It's Your Choice Leaflet This leaflet provides detailed information regarding online access and a list of things to consider before you access the service.
