Application to join the practice

Our practice list is open and we are currently accepting new patients. If you wish to register you can do so in person at the surgery or please complete the forms listed below.  To register you will need to live within the practice area (see below for the practice boundary).

Before being offered an appointment, all patients must be fully registered, either as temporary or permanent.  Any patient already registered with another practice in Bath will be redirected to their registered practice or advised to register as a permanent patient at The Pulteney Practice, if they so wish.  Reception will be able supply you with a brochure about the surgery, all of these details can also be viewed on the surgery website.

Please download & complete the new patient questionnaire in full. This is because it can take a considerable time for us to receive your medical records (Notes normally take 6 to 8 weeks to arrive from your previous surgery).  This form should be completed by all patients over the age of 16 and by those with complex medical issues requiring medication so that the doctor is aware in advance.

Once you have printed and completed the above documents, please hand these to reception on your first visit to the practice.

All patients wishing to register will need to provide 2 forms of identification per person, we will therefore request to see evidence of identity – preferably photographic (e.g. passport), AND evidence of your address in the UK in order to determine residence within the practice boundary.  Where this is not possible (for example for a young child), birth certificate and proof of address will be sufficient.  Please provide copies of the identification.

If you are from overseas and have not been registered in the UK before, you will also need to provide a Biometric Residence Permit or proof that you have a financial commitment to the UK before we can register you, e.g. a job (employment must be longer than 9 months), student (study must be more than 9 months), UK bank account statement, mortgage statement etc.

Please forward the forms as required to either of our surgery sites together with copies of your identity.  We aim to deal with your registration within 48 working hours.  Please therefore allow 2 working days before booking an appointment.  If we need any further information we will contact you accordingly.  The practice has discretion to accept anyone who applies or to refuse to accept anyone, provided there are reasonable grounds for doing so which are non-discriminatory.
